Pastoral Leadership in the Diocese of Crookston
Initial Scholarship Application Form

The purpose of this scholarship is...
to provide tuition assistance for lay persons and permanent deacons in
the Diocese of Crookston pursuing certification and/or a degree in some
area of church ministry; or pursuing continuing education in faith formation;
or enrolled in a
short term leadership formation program.

To be eligible, the applicant must:
- belong to a parish in the Diocese of Crookston
- give evidence of past and present parish commitment
- possess leadership qualities
- be available for church ministry in the Diocese of Crookston upon
completion of the program
- be enrolled or seeking enrollment in a program through a Catholic
institution that leads to certification and/or a degree in an area
of ministry including, but not limited to liturgy, catechetics,
pastoral ministry, leadership, etc.
or pursuing continuing education in faith formation in a Catholic
institution.
- submit completed application form with two letters of endorsement,
the first from your priest or pastoral administrator, the second
from a person familiar with you and your ministry.

Description of Ministry Goals - Compose
a concise essay about your educational and ministerial goals. Explain
how the program in which you are enrolled (or seeking to be enrolled)
will help you meet these goals. Describe your ministry in your present
church community and how you think the program will enhance your ministry.

Endorsements - Enclose with this form two letters of endorsement
of your proposal, one from your priest or pastoral administrator and
the second from a member of your parish or the diocese or someone
familiar with you and your ministry. Letters of endorsement should
refer to your:

as evidenced in your current parish ministry;
